Registrar

The Registrar is LSMSA's official record keeper and is available to assist with a variety of tasks associated with academic records, absence requests, and enrollment verification needs. The Registrar's office is responsible for establishing processes to support the LSMSA policies set forth in the Student Handbook. Students, alumni, and parents may find the links below helpful.

For all forms and requests, please allow 3-5 business days for processing.  Additional processing time may be required during holidays, advising/registration at the beginning of the semester, final grade processing, and/or unforeseen system problems.

  • College Credit Approval

    A student who wants to apply the credit from a college course toward their Louisiana School graduation requirements, or toward meeting prerequisites for a Louisiana School course, must obtain approval on the College Credit Approval Form prior to enrolling in the college course. After entering LSMSA, transferred credit will be limited to courses taken at a college or university in which a grade of B or better has been earned. This is true for a course taken at the student's initiative and for one being repeated as required by the school when a course has been failed.

  • Driver’s License/Learner’s Permit

    Effective 10/2021, Louisiana now requires a parent/guardian to attest to compliance of school attendance for minors applying for a drivers license or identification card.  

    A certification form from the school is no longer required.

    If you are obtaining a license in a state other than Louisiana, it is your responsibility to provide the appropriate state-approved form to the LSMSA Registrar for completion.
  • Employment Certificate for Minors

    Louisiana law requires that individuals under the age of 18 submit an a state-issued Employment Certificate to their potential employers.  LSMSA is not authorized to issue employment certificates but can assist with providing enrollment verification documents.

  • Enrollment Verification

    Students needing verification of enrollment for insurance or other purposes should submit an Enrollment Verification Request form to the Registrar's Office.  The form requires a signature.  Please allow 5 business days for processing.

  • Excused Absences

    Family Emergencies
    1. Requested through the Director of Student Support Services office, in consultation with the Director of Academic Affairs or the Director of the Living Community with as much advance notice as the situation allows.
    Personal Illness/Mental Wellness Concerns
    1. Managed through the School Nurse or the Personal Counselors.
    2. Excuses will not be allowed after a class (or portion of a class) has been missed. 
    School Related/Approved Functions
    1. Requested through Registrar’s Office, using the Excused Absence Form prior to registration or the payment of any fees associated with the function. This situation is often initiated by the faculty member/sponsor in charge of the event.            
    2. May not be allowed if on academic probation or if having academic problems, unexcused absences, or excessive excused absences.
    All Other Requests
    1. Requested by the student, 3 school days in advance, through the Registrar’s Office, using the Excused Absence Form on the Resource Board in myLSMSA.

    2. Transcript Request (LSMSA)

      Graduates or former students who need an official LSMSA transcript may request one using the form found at the link below.  Please allow 3-5 business days for processing.

    3. Transcript Requests (NSU)

      LSMSA students, graduates, or former students who participated in Dual Enrollment with Northwestern State University and who need an official NSU transcript should follow the instructions below for requesting their transcript from NSU.

    4. Withdrawal from LSMSA

      The withdrawal process begins in the Registrar’s Office.  Parents/guardians whose student(s) are withdrawing should meet with the Registrar during business hours (Monday-Friday 8:30-4:30) to complete the required forms.   The withdrawal form must be completed by parent/guardian before leaving the campus. Signatures verifying clearance with teachers, residence hall staff, Budget Office, and Director of Academic Affairs are required on the form.  Parents/guardians are encouraged to notify the Registrar as soon as they know their child will be withdrawing to allow sufficient time for withdrawal forms to be prepared.

      Under exceptional circumstances, families may need to move their student out of the residence hall on weekends or after hours.  In these rare cases, the Registrar will email withdrawal forms to the parent/guardian to be completed and returned to LSMSA within two business days after the student leaves campus.
